Laparoscopic Colorectal Surgery: What's new and better?

Laparoscopic SurgeryLaparoscopic or minimal access surgery in the abdomen has been around for the last 20 years. The first pioneering procedure was and still is commonly cholecystectomy or removal of the gallbladder. This is typically done as a day surgery or overnight procedure. Colon and rectal surgery has previously required a large incision in the abdominal wall to gain access. The end result has been post-operative pain, more medication required and a slower return of bowel function because of increased handling of the bowel during surgery.

Laparoscopic colorectal surgery typically requires 3 or 4 “ports” which measure 5-12mm in size to facilitate the introduction of instruments and a video camera system that allows surgery to take place. Technology has allowed many new energy sources to be used together with stapling devices that divide bowel and blood vessels. The specimen of resected colon is removed through a small 6-7 cm incision, which also allows reconnection of the intestine using sutures or staples.

Patients with laparoscopic procedures done recover up to 50 % faster and are discharged earlier. They also have lesser pain and are able to rehabilitate faster to return to work. Studies have shown that in skilled hands, there is no difference in the amount of tissue removed in cancer or recurrences comparing conventional and minimal access surgery.

Some of the common indications for laparoscopic colorectal surgery include cancer, diverticula’s disease, colon polyps and inflammatory bowel disease.
